She is holding a pack of Marlboro reds indeed and so is Fantasia from American Idol a well known Marlboro smoker. I first thought maybe a camera or cell phone but watching the one with Fantasia cleared it up since they are not being moved around like in Miley's hand. The size change is a camera trick it is very common when something is moving that the camera does not see it very well and creates artifacts aka a double image of the object.

Only caught a bit of the show with a man dancing... that male dancer who you showed a clip of dancing, he was in the audience... I'd like to see more of him dancing... hoped there was a clip on your web page.
